<p>I have been to two board-certified doctors in order to be evaluated for lasik/lasek/prk. My current prescription is:<br />
R SP -4.5 CYL -3.5<br />
L SP -3.0 CYL -3.25</p>
<p>cornea thickness and pupil diameter allow me to be eligible for either prk or lasek.</p>
<p>My questions are:<br />
Can the VISX CustomVue Laser using Custom Wavefront Technology be used to treat my prescription? </p>
<p>What amount of nearsightedness and astigmatism can this laser correct? </p>
<p>Can any other custom wavefront procedures work for me based on my astigmatism? I believe my astigmatism is myopic.</p>
<p>Based on my prescription, what procedure (Lasek or PRK) will likely produce the best result? </p>

<br />If your cornea thickness allows both, then I don&#8217;t think there is any issue with either.  PRK can be used on thinner corneas and if there needs to be a further correction, having PRK makes it more likely to be correctable if you have thinner corneas.  But these don&#8217;t sound like issues to you, since your corneas are fine.</p>
<p>I would suggest PRK.  The reasons to do lasik over PRK is that it heals faster, is less painful, and you get instant results.  PRK takes a few weeks to get full results (though it will continue to improve for up to a year).  And it can be painful for the next couple of days.  Also, you will not be able to see well enough to drive or even work for a several days.  But, PRK has fewer risks (they can&#8217;t over cut the flap) and in the long run there are fewer risks since there is no flap to cause problems.  (LASIK flaps can come un-seated years afterward if you get hit in the eye, since it never really heals.)  Also, PRK tends to improve over the long run, to give you better results.  I had PRK and the pain was not significant and I had could take a week off from work.  I would say the pain and the time off was totally worth the reduced risk.  In my mind, LASIK is for people who are afraid of the pain and want instant results.  But PRK is a better procedure with less risk and better results.  If it were me, I would go for better results and less risk.  And that&#8217;s what I did.  I am happy with my decision.</p>

			<content:encoded><![CDATA[<p>I had laser surgery (LASIK) in 1999 in Chester (UK). I&#8217;ve been back and forth the clinic (and the Cardiff clinic over the last few years) to this day (almost 10 years later). I had further treatment to my right eye in 2005 (Wavefront), which made my vision worse still.<br />
After 10 years of no improvement from numerous visits to the Ultralase clinic, I&#8217;m wondering whether or not to seek legal advice (something I was advised by a friend to do many years ago). I am happy to discuss the extent of my side effects in a personal e-mail if you give me your e-mail address.<br />
So I guess, my question comes in 2 parts, 1 &#8211; do you think I should look into taking legal action? and 2 &#8211; what other advice would you have for me?</p>

<br />LASIK is not always permanent to correct vision and there are some side effects that you pretty much sign away on before you have the surgery.  You can seek legal advice, but I&#8217;m betting the contract you signed convered all their bases to protect them.</p>

			<content:encoded><![CDATA[<p>Went to a laser eye consultation today, my pupil size is 9.0mm, the largest they had ever recorded.  They said I could have lasik with wavefront. </p>
<p>I am concerned that an area of only 6.5mm is corrected to my prescription of -4.5, meaning an outer area of 2.5mm in dark conditions will be receiving, but NOT focussing light onto my retina at the correct prescription.  I fear that car lights at night will scatter light on my retina causing haloes, or starbursts, is this a fair assumption based on the physics of optics?  Would you take the risk?<br />
<br />If you do a quick search on google about large pupils and LASIK you&#8217;ll see a lot of doctors hesitant to perform the procedure even on patients with milder prescriptions than yours.  You&#8217;ll also see accounts of people with large pupils who have had the procedure done and find those starbursts and halos debilitating.  </p>
<p>I personally would not take the risk.  I&#8217;m not an ophthalmologist, just an optician who had LASIK a few months ago, so keep that in mind, but there is a very real risk for some very serious side effects in your case.  The technology used for LASIK is always evolving.  They may come out with a procedure more suitable to your situation in the future.  There are also other procedures/techniques to consider now, like Ortho-K.  Glare and halos are also a potential side effect with this process but, unlike LASIK, it&#8217;s reversible.</p>
<p>If you are still considering the LASIK I would absolutely get another opinion.  I&#8217;d actually get 2-3.  See what other doctors say about your situation.  If you only find one doctor who will do it that should tell you something.</p>

<br />Hi there,</p>
<p>2 weeks on and still inflamed after LASIK surgery &#8211; interesting (did she have Intralase or Femtosecond flaps cut?). Do you have more detail regarding what her treated prescription was and who (company and consultant) were, her age, specific plan of treatment (distance correction or monovision?). Use of anti-biotic drops and or anti-inflammatory drops at this stage are also more typical of treatments involving Femtosecond lasers for flap creation. Is she suffering an acute aversion to light? </p>
<p>Dry eyes are common, sometimes causing the epithelium to rough up and blur vision. DLK is a distinct possibility depending on who has done the treatment, they normally attempt to medicate with Pred Forte or other strong anti-inflammatories. </p>
<p>Short of seeing her eyes, I can&#8217;t comment with much accuracy. Do email me with as much info as you can and I&#8217;ll attempt to be as helpful as possible.</p>

<p>If you&#8217;re like me, you&#8217;ve probably heard about some of the side effects associated with getting lasik vision correction surgery. For instance, after getting lasik surgery some people complain about symptoms of glare, halos and decreased night vision.</p>
<p>In fact, I remember attending a business briefing where the speaker started by mentioning he just had lasik surgery the day before. He told the audience, &#8220;The good news is I think you&#8217;re all going to heaven because everyone has halos around their heads!&#8221;</p>
<p>If you&#8217;ve considered getting lasik surgery but were concerned about some of the less-than-desirable side effects, you&#8217;ll be glad to hear that new technology is reducing those dreaded halos and glare. Known as &#8220;custom&#8221; or wavefront-guided lasik, this relatively recent improvement in eye surgery technology produces better vision quality when compared to traditional lasik vision surgery.</p>
<p>&#8220;Most significant,&#8221; says US Navy Captain Steve Schallhorn, MD, the Director of Cornea and Refractive Surgery at the Navy Medical Center, San Diego, &#8220;is the improved quality of vision with the wavefront-guided procedure, fewer problems with halos and glare, better night vision, and higher patient satisfaction.&#8221;</p>
<p>Traditional lasik reshapes the patient&#8217;s cornea with a laser to correct visual problems like nearsightedness or farsightedness. With conventional lasik, the laser is guided based on formulas similar to the ones used to determine your eyeglass prescription. Conventional lasik is effective at treating &#8220;lower-order&#8221; aberrations like nearsightedness, farsightedness and astigmatism (also called &#8220;refractive errors&#8221;).</p>
<p>Wavefront-guided lasik, on the other hand, measures how light is distorted as it passes into the eye and then is reflected back. This creates a 3-D optical map of the eye, highlighting the imperfections and disorders that lead to visual errors. Wavefront technology then creates a custom treatment plan for each eye and guides the laser during the surgery. </p>
<p>Wavefront-guided lasik can effectively treat the same lower-order aberrations that conventional lasik can treat as well as treating &#8220;higher-order&#8221; aberrations such as decreased contrast sensitivity, night vision, glare, shadows and halos. In fact, there are a growing number of patients turning to wavefront-guided lasik to correct side effects from previous laser eye surgeries.</p>
<p>&#8220;With this technology breakthrough, we can now measure these disorders, show the patient what&#8217;s going on in their eye, link that information to the laser, and actually correct higher-order aberrations,&#8221; says Roger Steinert, M.D., associate clinical professor of ophthalmology, Harvard Medical School. &#8220;Wavefront technology enables the surgeon to improve overall vision quality better than in the past.&#8221;</p>
<p>Of course, this advanced technology comes with a higher price tag than traditional lasik surgery. And just like conventional lasik, wavefront-guided lasik is a surgical procedure and has risks associated with it. Be sure to consult with a qualified lasik surgeon to discuss potential complications and ensure you&#8217;re a good candidate for this type of surgery.</p>
<p>But if the choice is between crystal clear vision with no side effects and going through life seeing all the people going to heaven, I think I&#8217;ll go with the first option, please!</p>

<br />There are three main treatment choices for anyone thinking of having laser eye surgery. But which one will be right for you?</p>
<p>The final choice will be determined between you and your laser eye surgeon. BUT, of course, you have the eyes and your surgeon has the expertise so you will be heavily influenced by what the doctor or another clinic staff member says. It will be better to be prepared. With just a little knowledge you can be actively involved in making the right choice.</p>
<p>I will outline the 3 main options to help you have an informed discussion when the time comes to select which approach is best for your individual eyes. After all, you only have one pair so you want to get it right.</p>
<p>You will be overwhelmed at first by all the terminology, treatments and brand names. Don’t worry. There is no need to be blinded by science or some of the frankly misleading marketing information that is out there.</p>
<p>Firstly, whichever of the 3 treatment types (more on these coming in a couple of days in part 2) you go for, you must go for the wavefront option. The wavefront (or custom) technology has made a huge improvement in vision success rates with many more people getting 20/20 sight or even better.</p>
<p>Different clinics use different names for this technology: custom, wavefront, wavefront guided, wavefront optimised, CustomVue.</p>
<p>Also wavefront laser eye treatment will give better vision at night or in low light situations. Patients will notice less halos and glare from light sources such as car headlights. It is like buying a more expensive camera with a much, much better lens.</p>
<p>This is the year 2008, so get the best current technology for your sight correction &#8211; you only will have this done once and you will live with the results for the rest of your life. Think about the computer you had back in 2000 and what is now available in 2008 &#8211; that is the level of difference in laser technology too. No brainer, really.</p>
<p>If you can’t afford the price, either don’t have it done yet and save up until you can afford it, or think about financing.</p>

<p>Wavefront technology takes 3D images of your eye to help guide in the reshaping process of your cornea. This affords ophthalmologists an ability to achieve a higher degree of precision in surgical corrections than was ever possible in the past.</p>
<p>While glasses, contact lenses, and older <a href="http://www.lasikdocs.com/lasik.html">LASIK procedures</a> could only correct for lower order aberrations (myopia, hyperopia, and astigmatism), wavefront technology has the ability to correct for higher order aberrations (halos, glare, shadows, blurry images) as well.</p>
